# Men's Health After 40

					## What Actually Changes, What the Numbers Say, and What to Check

					### Highlights

									- **Feeling tired, heavier or slower to recover after 40 is common, and it is rarely just one thing. **Metabolic health, heart risk, sleep, stress, nutrition and hormones all play a part, and more than one is usually involved.
- **It is rarely just testosterone. **In the largest European study, only about 2 in 100 men aged 40 to 79 had low testosterone with symptoms. Most men keep normal levels into later life.
- **You can feel well while risk is building. **In the UAE, about 1 in 5 adults has diabetes and most cases are undiagnosed. The right tests at the right time show you where you stand.

					### Quick Answer

If you are a man over 40 who feels more tired, carries more weight around the middle, recovers more slowly or has noticed changes in libido, sleep or focus, several things may be happening at once. Metabolic health, cardiovascular risk, sleep, stress, nutrition, hormones, medications and other medical conditions can all contribute. At Menovivre, a physician-led men’s health clinic in Dubai, assessment begins with a detailed consultation, followed by testing chosen for your individual picture, and a plan that is measured over time. Menovivre is not a urology clinic: where specialist urological or prostate care is needed, we refer you to the right expert.

You still train. You still work hard. But something has changed, and you are not sure whether to put it down to age, stress or something worth checking. Many men in Dubai feel the same, and most do not come to us because they are unwell. They come because they want to know what is actually changing, and what to do about it. This page explains what changes after 40, what the numbers say and the care that can help, with every figure linked to its source.

					## What changes after 40?

**Your muscle and metabolism. **Muscle becomes harder to build and keep, and weight settles more easily around the abdomen. Two men of the same weight can have very different amounts of muscle and visceral fat, and very different metabolic risk. This is why weight alone tells you very little.

**Your heart. **Blood pressure, cholesterol particles and blood sugar can drift for years before anything feels wrong. Checking them early is one of the kindest things you can do for your future self.

**Your hormones. **Testosterone can fall gradually with age, but a large longitudinal study found that once other conditions such as obesity and diabetes were accounted for, age itself did not predict a significant decline. Low testosterone is a diagnosis, not an assumption.

**Your sleep and recovery. **Poor sleep, work stress, travel and long hours affect energy, appetite, concentration and how well you recover from exercise. They are often part of the picture, and they are treatable.

					## Is it testosterone?

Sometimes, but less often than you might expect. In the European Male Ageing Study of 3,369 men aged 40 to 79, [late-onset hypogonadism](https://pubmed.ncbi.nlm.nih.gov/22419720/) (low testosterone with symptoms) affected about 2.1% overall: 0.1% of men in their forties, 0.6% in their fifties, 3.2% in their sixties and 5.1% in their seventies. Read the other way round, more than 7 in 10 men keep normal testosterone levels into later life.

That is why [Endocrine Society guidance](https://pubmed.ncbi.nlm.nih.gov/29562364/) asks for symptoms plus consistently low morning results before low testosterone is diagnosed. One reading is not a diagnosis. Our article on [normal blood tests when you still feel unwell](https://menovivre.ae/insights/normal-blood-tests-but-still-feel-unwell/) explains why a single result rarely tells the whole story.

					### Men's health after 40: the numbers

Numbers like these can feel stark, so read them the other way round too: most men over 40 do not have low testosterone, and most risk that builds quietly can be found early and managed.

        Measure
        What the research shows

        Lipoprotein(a)
Elevated in about 1 in 5 people worldwide; an inherited risk factor that a standard cholesterol test does not include
        European Atherosclerosis Society, 2022

        Diabetes in the UAE
20.7% of adults; 64% of cases undiagnosed

					### Which tests should a man have after 40?

It begins with a detailed consultation, not a test menu. Your physician reviews your symptoms, medical and family history, medications and supplements, sleep, nutrition, exercise, body composition, sexual health where relevant, and your health and performance goals. Testing is then selected to fit your picture.

Area
What is measured
What it tells you

Metabolic health
Fasting glucose, HbA1c, fasting insulin
Insulin resistance and prediabetes, before they show up as disease

Cardiovascular risk
Fasting lipid profile, ApoB, Lipoprotein(a), hs-CRP, homocysteine, blood pressure
Risk that a standard cholesterol check can hide

Thyroid
TSH, free T4, free T3
Whether thyroid function is contributing to fatigue or weight change

Hormone health
Total and free testosterone, SHBG, free androgen index, DHEA-S, morning cortisol
Interpreted with your symptoms, never in isolation

General health and nutrition
Liver panel, kidney function, electrolytes, uric acid, ferritin, folate, B12, vitamin D, zinc, copper, magnesium
Deficiencies and organ function that affect energy and recovery

Male-specific markers, where appropriate
Total and free PSA
With referral to a urologist if anything needs specialist follow-up

Depending on your results and goals, we may also recommend continuous glucose monitoring, or arrange DEXA scanning and VO2 max testing through our diagnostic partners. The point is not whether one value sits outside a reference range. It is to bring the findings together with your symptoms, history and objective data, and build a plan from that.

					### How does care change after 40?

									If you feel fine

You can feel fit and well while risk is already building. We look beyond weight alone and assess glucose, HbA1c, fasting insulin, lipids, ApoB, Lipoprotein(a), hs-CRP and blood pressure, together with your history and body composition. This matters most if you carry weight around the abdomen, have a family history of heart disease or diabetes, have abnormal cholesterol or high blood pressure, or have lost fitness.

If you want to build or keep muscle

We look at body composition, muscle health, metabolic markers and cardiovascular fitness rather than kilograms lost. Where objective measurement helps, DEXA scanning and VO2 max testing can be arranged through our partners.

If you want to build or keep muscle

Weight management starts with why. We assess metabolic health, insulin resistance, body composition, nutrition, activity, sleep, medications and hormones before deciding what to do. Treatment may include [nutrition](https://menovivre.ae/our-services/functional-nutrition-menopause-dubai/) and lifestyle changes, treatment of underlying metabolic problems, and weight-management medication such as GLP-1 therapy where clinically appropriate. The aim is better metabolic health and body composition, with muscle and physical function protected, not just a lower number on the scale.

If your libido or sexual performance has changed

Yes, we can help, within the scope of our physicians. Changes usually have more than one contributing factor and should not be assumed to be low testosterone. Hormones, metabolic and cardiovascular health, medications, sleep and stress may all play a part. Our physicians carry out an initial assessment and treat the medical, metabolic or hormonal factors involved. Where symptoms need specialist urological investigation or treatment, we refer you to a urologist.

					## What is different about men's health in Dubai?

Three local points are worth knowing.

**Diabetes and prediabetes are common here.** The [IDF Diabetes Atlas](https://diabetesatlas.org/data-by-location/country/united-arab-emirates/) estimates that 20.7% of adults in the UAE have diabetes, and that 64% of cases are undiagnosed. In the 2019 Dubai Household Health Survey, [prediabetes affected 20.5% of men](https://knepublishing.com/index.php/ddej/article/view/17035/26963). A fasting glucose, HbA1c and fasting insulin test can show where you stand.

**Excess weight is common too.** In the same survey, [41.5% of men in Dubai were overweight and 16.6% had obesity](https://bmjopen.bmj.com/content/13/1/e062053), rising to 23.2% with obesity between 40 and 49. Long hours, air-conditioned days and limited outdoor activity in the hotter months all make it easier to lose muscle and gain fat.

**Many men hear about us from their partner.** Some first come to Menovivre after their wife or partner has been treated here, while others already know one of our physicians. Men are seen by the same team.

The objective is the right expertise, whether it sits within Menovivre or elsewhere.

Testing supports clinical judgement, and progress is measured over time. The aim is simple:

									01

									Understand

									where you are today

									Identify

									what needs attention

									03

									Intervene

									where appropriate

									04

									Measure

whether your health is actually improving

					## Frequently Asked Questions (FAQs)

Q: Why am I tired even though my routine blood tests are normal?

A: Routine panels check a limited set of markers against broad reference ranges. Fatigue can come from insulin resistance, poor sleep, nutrient deficiencies, thyroid changes, hormone patterns or stress physiology that a basic panel does not show. We look at the wider picture and interpret results alongside your symptoms.

Q: Why am I gaining belly fat even though I exercise?

A: Abdominal fat is driven by more than calories. Insulin resistance, sleep, stress, hormones, muscle loss and medications all influence where fat is stored. Assessment looks at metabolic health and body composition rather than weight alone, so the plan addresses the cause.

Q: Should I have my testosterone checked?

A: If you have symptoms such as low libido, fatigue, loss of muscle or low mood, testosterone is worth checking as part of a broader assessment. Low testosterone is only diagnosed when symptoms are matched by consistently low morning results, and other causes are considered first. One reading is not a diagnosis.

Q: Is low testosterone causing my low libido?

A: Possibly, but not necessarily. Libido and sexual performance are affected by hormones, metabolic and cardiovascular health, medications, sleep and stress. We assess all of these. Where specialist urological investigation is needed, we refer you to a urologist.

Q: What health tests should a man have after 40?

A: A baseline assessment typically covers metabolic markers (glucose, HbA1c, insulin), cardiovascular risk (lipids, ApoB, Lipoprotein(a), hs-CRP), thyroid, hormones, nutritional status and, where appropriate, PSA. Which tests you need depends on your symptoms, history and goals.

Q: My cholesterol is normal. Do I still need ApoB or Lipoprotein(a)?

A: A normal standard cholesterol result can still miss risk. ApoB measures the number of particles that cause plaque, and Lipoprotein(a) is an inherited risk factor that about 1 in 5 people carry and a routine panel does not include. European guidance recommends measuring it once in every adult.

Q: Should I have a DEXA scan or VO2 max test?

A: They are useful when an objective measure of body composition or cardiovascular fitness would change your plan or help track progress. Neither is routine for everyone. Where indicated, we arrange them through our diagnostic partners.

Q: Can I use a GLP-1 medication without losing muscle?

A: Muscle loss is a recognised risk with rapid weight loss on any approach. We prescribe GLP-1 therapy only where clinically appropriate, and always alongside adequate protein, resistance training and monitoring of body composition so that fat is lost and muscle is protected.

Q: I am generally healthy. What should I be doing now to stay that way?

A: Establish a baseline, understand your metabolic and cardiovascular risk, protect muscle through strength training and adequate protein, prioritise sleep, and review the numbers at regular intervals. Prevention is easier than correction.
